If your dog was just diagnosed with heart disease, "switch to low sodium dog food" was probably one of the first pieces of advice you heard. It sounds simple. It is not.

Here's what makes it complicated: most dog food labels don't tell you the sodium content in any usable way, the definition of "low" depends on your dog's disease stage, and restricting sodium too aggressively at the wrong time can actually work against you. Meanwhile, the foods specifically designed for cardiac dogs are expensive, and a fair number of dogs flatly refuse to eat them.

Early-stage disease: don't over-restrict

A dog with preclinical disease - Stage B1 or B2 mitral valve disease (MMVD) or occult DCM - is not retaining fluid. At this stage, veterinary cardiology guidance generally calls for avoiding high-sodium foods and treats, not pursuing aggressive restriction.

There's a physiological reason for this caution: severe sodium restriction can activate the renin-angiotensin-aldosterone system (RAAS) - the hormonal cascade that tells the body to hold onto sodium and water. RAAS activation is exactly what several cardiac medications are designed to suppress. Restricting sodium hard in a stable, asymptomatic dog can push the body in the wrong direction. More restriction is not automatically better.

Practical translation for Stage B: feed a good-quality commercial diet that isn't sodium-heavy, cut the salty extras (deli meat, cheese, jerky, pizza crust), and save the strict measures for if and when they're needed.

Congestive heart failure: this is where it counts

Once a dog has been in congestive heart failure (Stage C), moderate sodium restriction becomes a standard part of management alongside diuretics and other medications. The goal is to reduce the sodium load the diuretics have to fight, without restricting so severely that the dog stops eating or RAAS activation worsens.

Advanced disease: palatability wins

In refractory failure (Stage D), tighter restriction may be discussed - but appetite usually becomes the limiting factor. A dog that refuses the low-sodium food gets zero benefit from it and slides toward cardiac cachexia. At this stage, most cardiologists prioritize keeping the dog eating over hitting a sodium number.

Two rules that apply at every stage

  1. Never switch diets abruptly. A sudden drop in sodium intake can cause shifts in fluid and electrolyte balance. Transition over 7-14 days.
  2. Consistency beats perfection. A moderately low-sodium diet the dog eats reliably outperforms a strict diet fed inconsistently and topped up with salty bribes.

How to actually read sodium on a dog food label

This is where most owners get stuck, because dog food labels seem designed to hide the answer.

The guaranteed analysis usually won't help you

Look at the bag: you'll see protein, fat, fiber, and moisture percentages. Sodium is often missing entirely, and when it does appear, it's typically a minimum percentage - which tells you the least amount that could be in there, not how much actually is. A "minimum 0.3% sodium" food could contain considerably more.

Percentages are also misleading on their own because they don't account for moisture (canned food looks "lower" than kibble on paper) or calorie density. Two foods with the same percentage can deliver very different sodium loads per meal.

The unit that matters: mg per 100 kcal

Veterinary nutritionists express sodium as milligrams per 100 kilocalories (mg/100 kcal), sometimes written as mg/1000 kcal - just move the decimal. This normalizes for moisture and calorie density, so you can compare a dense kibble against a watery can fairly. Since dogs eat to meet calorie needs, it also reflects what your dog actually consumes.

The math, if you ever have the raw numbers: take the sodium in mg per kg of food, divide by the food's kcal per kg, multiply by 100. In practice you'll rarely need to do this yourself, because of the next trick.

The manufacturer-call trick

Most companies don't print mg/100 kcal anywhere - but they have it. Call or email the manufacturer (the number is on the bag) and ask one specific question:

"What is the sodium content of [exact product name and formula] in milligrams per 100 kilocalories?"

Ask for the typical or average analysis, not the guaranteed minimum. A reputable manufacturer answers this within a day or two. A company that can't or won't tell you the sodium content of its own food has told you something useful too.

Keep a note on your phone with the numbers for the foods you're considering, and bring it to your cardiology appointments.

What actually counts as "low sodium"?

There's no legal definition of "low sodium" on a dog food label, so the marketing term means nothing. The veterinary nutrition literature, however, describes general ranges - and it's worth stressing these are ranges discussed in that literature, not rules, and your cardiologist may set a different target:

  • Typical adult maintenance foods commonly run well above 100 mg/100 kcal - often several times that. This is fine for healthy dogs.
  • Mild-to-moderate restriction, often discussed for dogs in or approaching heart failure, is roughly in the neighborhood of 80-100 mg/100 kcal.
  • Marked restriction - figures below that range, sometimes cited around 50-80 mg/100 kcal - is generally reserved for advanced disease under direct veterinary supervision, because of the RAAS and palatability trade-offs above.

The practical takeaway: for a heart dog, you're usually trying to get out of the high range and into a moderate one, not chasing the lowest possible number. Get the target from your cardiologist, then use the mg/100 kcal figures you've collected to find foods that hit it.

The prescription options

A handful of therapeutic diets are formulated specifically for cardiac patients, with controlled sodium plus additions like taurine, carnitine, and omega-3 fatty acids:

These diets have real advantages: the sodium content is known and controlled, the formulation work is done by veterinary nutritionists, and your cardiologist knows exactly what your dog is eating.

They also have the two drawbacks that dominate every owner forum thread about them: cost and refusal. These are premium-priced foods, and feeding a large-breed dog one long-term is a genuine budget line item. And some dogs - after a few days or a few months - simply stop eating them. The OTC route

This tension comes up constantly, and both sides have a point, so let's handle it fairly.

Why cardiologists recommend what they recommend: a therapeutic diet is a known quantity. Sodium, taurine, protein, calories - all controlled and verified. When a cardiologist is titrating diuretics against fluid retention, an unknown and variable sodium intake makes their job harder and your dog's management less precise. Their recommendation isn't brand loyalty; it's risk control.

Why owners push back: some dogs genuinely eat better, or have done well for years, on raw or fresh diets. Palatability is not a trivial concern in a disease where appetite loss is a major threat. And "feed this bag forever" can feel like a downgrade from a diet you've invested real care in.

The honest middle ground: raw and home-prepared diets are not automatically off the table for cardiac dogs, but "not off the table" is doing a lot of work in that sentence. Unbalanced homemade diets have been implicated in nutritionally mediated heart disease, sodium content of a DIY raw diet is essentially unknown without formulation, and food-safety risks carry more weight in a chronically ill dog.
