Enlarged Heart in Dogs: What It Means and What Happens Next
Your vet said your dog's heart looks enlarged on an X-ray. Learn what that actually means, the possible causes, why an echo comes next, and what treatment looks like.

First, take a breath
Maybe your dog went in for a cough. Maybe it was a routine X-ray before a dental cleaning, or a workup for something completely unrelated. And then your veterinarian pointed at the image and said the words that sent you straight to a search engine: "The heart looks enlarged."
If you are reading this in the hours after that appointment, here is the most important thing to understand: an enlarged heart is a finding, not a diagnosis. It is a clue that something is affecting the heart - but it does not tell you what, how severe it is, or what your dog's future looks like. Those answers come from the next step, and for many dogs, the news at that next step is far better than the panic of this moment suggests.
This article walks through what "enlarged" actually means, the conditions that can cause it, why an echocardiogram is the essential next test, and how to think about treatment and life expectancy honestly - because both depend entirely on the cause.
What "enlarged heart" actually means
The medical term is cardiomegaly - literally, "big heart." But hearts do not enlarge in just one way, and the pattern of enlargement matters enormously.
Dilated: the chambers stretch
In some diseases, the heart's chambers - the atria and ventricles - stretch and balloon outward. The walls may actually become thinner as the chamber expands, like an overinflated balloon. This is the pattern seen in dilated cardiomyopathy (DCM), where a weakened heart muscle cannot pump effectively and the chambers dilate in response.
Thickened: the walls grow inward
In other conditions, the heart muscle itself thickens (hypertrophy). The outside of the heart may look bigger while the chambers inside actually get smaller. This pattern is more classically a cat problem (hypertrophic cardiomyopathy), but in dogs it can occur secondary to conditions like pulmonic or subaortic stenosis, or chronic high blood pressure.
Volume overload: the heart compensates
The most common pattern in dogs is enlargement from volume overload. When a valve leaks - as in mitral valve disease - some blood flows backward with every beat. The heart has to handle that extra recirculating volume, so the affected chambers gradually stretch and remodel to compensate. The enlargement here is not the disease itself; it is the heart's response to the disease.
These patterns look similar on an X-ray - a bigger cardiac silhouette - but they mean very different things, require different treatments, and carry very different prognoses. Which is exactly why an X-ray alone cannot finish this conversation.
The possible causes
Myxomatous mitral valve disease (MMVD)
In small and medium breeds - Cavaliers, Dachshunds, Chihuahuas, Poodles, Yorkies, and many mixes - the most common cause of an enlarged heart is MMVD, a slow degeneration of the mitral valve. As the valve leaks, the left atrium and left ventricle enlarge to handle the regurgitated blood. This is by far the most frequent explanation for cardiomegaly in an older small-breed dog, and it is also the cause with some of the best-established treatment evidence in veterinary cardiology.
Dilated cardiomyopathy (DCM)
In large and giant breeds - Dobermans, Great Danes, Irish Wolfhounds, Boxers, and others - an enlarged heart may signal DCM, a disease of the heart muscle itself. The weakened muscle contracts poorly, and the chambers - especially the left ventricle - dilate. DCM can also occur in atypical breeds, sometimes linked to diet or taurine deficiency, which matters because some nutritional forms are partially reversible.
Less common causes
- Congenital defects - conditions like patent ductus arteriosus (PDA) create abnormal blood flow that enlarges specific chambers, often detected in young dogs.
- Heartworm disease - advanced infections enlarge the right side of the heart.
- Pericardial effusion - fluid in the sac around the heart can make the silhouette look large and round on an X-ray even when the heart muscle itself is normal.
- Arrhythmia-driven enlargement - chronic rhythm disturbances can gradually remodel the heart.
- Athletic heart - very fit, active dogs (think sled dogs and sighthounds) can have mildly larger hearts that are completely normal for them.
That last point deserves emphasis: some breeds and body types simply have hearts that sit at the larger end of normal. Deep-chested and barrel-chested dogs can also appear to have enlarged hearts on X-ray because of how the chest is shaped. Radiographic "enlargement" is sometimes not enlargement at all.
The VHS: how vets measure heart size on an X-ray
Your veterinarian may mention a vertebral heart score (VHS) - and it is worth understanding, because it is simple and clever.
On a side-view chest X-ray, the vet measures the length and width of the heart, then compares those measurements against the dog's own spine, counting how many vertebrae the heart "spans." Because the measurement is scaled to the individual dog's body, it works across a Chihuahua and a Great Dane alike.
- A VHS of roughly 8.5 to 10.5 is considered normal for most breeds.
- Above about 10.5 to 11, the heart is generally considered enlarged - though some breeds (Boxers, Bulldogs, Cavaliers, Labradors, Whippets) have naturally higher normal ranges.
- The higher the number, and the faster it climbs between X-rays, the more significant the enlargement.
The VHS is a screening measurement, not a verdict. It tells your vet the heart is bigger than expected - it cannot say which chamber, why, or how well the heart is functioning. You can read more about what X-rays can and cannot show in our guide to the chest X-ray.
Why the X-ray is not the diagnosis - and the echo is the next step
Here is the mental model that will serve you best: the X-ray raises the question; the echocardiogram answers it.
A chest X-ray shows the heart's outline - its shadow. It is excellent at spotting overall enlargement, evaluating the lungs, and detecting fluid buildup (which is why it remains the go-to test when heart failure is suspected). But a shadow cannot show which chamber is enlarged, whether the walls are stretched or thickened, whether the valves are leaking, or how strongly the muscle is contracting.
An echocardiogram - a cardiac ultrasound, ideally performed by a veterinary cardiologist - shows all of that in real time. It measures each chamber precisely, watches the valves open and close, uses Doppler to visualize leaks, and quantifies how well the heart is pumping. It is non-invasive, painless, usually done without sedation, and typically takes 20 to 45 minutes.
After the echo, you will have what you do not have today: an actual diagnosis, a severity assessment, and a plan. Do not skip this step. Treating an enlarged heart without knowing the cause is guessing - and the right treatment for one cause can be the wrong treatment for another.
Treatment depends entirely on the cause
There is no medication "for an enlarged heart." There are treatments for the diseases that enlarge hearts, and they differ:
- MMVD: For dogs whose hearts have enlarged past a defined threshold (Stage B2), the landmark EPIC study showed that pimobendan delays the onset of heart failure by a median of roughly 15 months. - Pericardial effusion: Treated by draining the fluid and addressing why it accumulated - a completely different pathway from valve or muscle disease.
- Heartworm: Treated with an established adulticide protocol.
Notice what all of these have in common: the treatment targets the cause, not the size. This is the practical reason the echocardiogram matters so much.
Life expectancy: an honest framing
This is probably the question that brought you here, so let's address it directly - without false reassurance and without doom.
There is no single life expectancy for "enlarged heart," because enlargement is not one disease. The honest answer has three parts:
- Cause matters most. A small-breed dog with mild enlargement from early MMVD may live for years - often a normal lifespan - with monitoring and, when indicated, medication. A Doberman with advanced DCM faces a more serious prognosis. These are not the same conversation.
- Stage matters almost as much. Within every cardiac disease, dogs diagnosed before symptoms appear do meaningfully better than dogs diagnosed in heart failure. An enlarged heart found incidentally on a routine X-ray, in a dog with no symptoms, is very often an early or middle-stage finding - which is genuinely good news, because it means there is time to act.
- Treatment changes the numbers. Studies report that appropriately timed medication delays heart failure in both MMVD and preclinical DCM by many months to over a year. The dogs who benefit are the dogs who got diagnosed - which is what you are in the process of doing.

One more honest note: some dogs with radiographic "enlargement" turn out to have normal hearts on echo. That outcome is common enough that it is worth holding as a real possibility until the echo says otherwise.
Questions to ask at your next appointment
Walking into the follow-up with questions written down helps - both for your understanding and for your composure. Consider asking:
- Which part of the heart looks enlarged, and what was the VHS?
- Could my dog's breed or chest shape explain some of this?
- Do you suspect a specific cause based on breed, age, and exam findings?
- When can we get an echocardiogram, and will it be with a board-certified cardiologist?
- Are there any symptoms I should watch for in the meantime - and what would count as an emergency?
- Should I start counting my dog's resting breathing rate at home? (The answer is almost always yes: count breaths per minute while your dog sleeps; consistently over 30 warrants a call.)
- Is it safe to continue normal walks and play until the echo?
- If medication is eventually needed, what would the goals of treatment be?

Frequently asked questions
Can a dog live a normal life with an enlarged heart?
Many can - it depends on the cause and stage. Dogs with mild enlargement from early valve disease frequently live normal, active lives for years, sometimes never progressing to symptoms. Dogs with enlargement from advanced muscle disease face a harder road. The echo determines which situation your dog is in, and even in the more serious scenarios, treatment meaningfully extends good-quality time.
Is an enlarged heart in dogs always serious?
No - but it always deserves follow-up. Some radiographic "enlargement" reflects breed conformation, athletic conditioning, or chest shape rather than disease, and some reflects early-stage conditions that need monitoring rather than treatment. The finding is serious enough to investigate and common enough that investigation often brings reassuring news.
How fast does an enlarged heart progress?
It depends on the cause. MMVD typically progresses slowly, over years. DCM can progress faster, over months to a couple of years. This is another reason the diagnosis matters: it tells you what monitoring schedule makes sense.
Can an enlarged heart go back to normal?
Sometimes. Diet-associated DCM can partially or substantially reverse with diet change. Enlargement from a treatable cause (heartworm, some congenital defects, pericardial effusion) can improve once the cause is addressed. Enlargement from degenerative valve disease generally does not reverse, but its progression can be slowed.
What are the symptoms of an enlarged heart in dogs?
Often none at all - which is why so many are found incidentally. When symptoms do appear, the common ones are coughing (especially at night), faster breathing at rest, tiring easily on walks, fainting episodes, and reduced appetite. Any of these alongside a known enlarged heart warrants prompt veterinary attention.
The bottom line
"Enlarged heart" is the beginning of a diagnostic process, not the end of one. The X-ray told your vet that something deserves a closer look; the echocardiogram will tell you what it is, how significant it is, and what - if anything - needs to be done.
